What is the application process?
First, you will need to gather the following information:

- Report of Separation (DD from 214) – received upon Discharge from service
- Copy request info:
- National Archives & Records Administration

- Allow 1 – 3 months processing time
- Marriage License (former & current)
- Death Certificate (if surviving spouse is applicant)
- Social Security number for each application (Veteran & Spouse)
- Name & contact information of child/family member/overseer of application’s personal affairs
- Statement from care taker as to related expenses (monthly basis)
- Proof of current income amount (social security, retirement)
